Headset always comes loose

Hello everyone.  I’ve been having a lot of trouble with this headset.  I tighten it, ride for a minute, and it comes loose every time.  I did not install this.  It came on the bike, which I got in a trade.  Thanks in advance for any advice. Here a few pics:

Ok.  So you are thinking that washer is stripped OR the slot on the forks themselves? 

Of course I will have to disassemble and figure this out, but wanted to throw this out to the forum and see what everyone thinks.  I have a video but cannot figure out how to load it from my iPhone.

Key slots don't strip all the way around, but if they strip a little you can get enough wiggle room for the thing to work itself loose.

more likely last owner put the spring in up side down. so head set do's not work wright. spring holds it all tight. put it in up side down. it will try to loosen it.  tongue  so flip it the other way.

I am doing a quick dissection of this headset.  First, here is the key washer:

Next, here is the internal spring set up:

Third, here is a close up of the gap between the headset cup and the threaded bottom of the headset (the spring housing)——is this gap supposed to be there? It’s tightened as far as I can tighten it.

Finally, when I turn the wheel to the left the bottom part (the black part that houses the spring in the last pic) tightens.  When I turn the wheel to the left it loosens.  In other words, the spring housing (black part) moves with the fork/wheel.  Is this supposed to do that?

No the spring seems to be where it’s supposed to and doesn’t grab onto anything except the notches in the housing piece.

I tightened the black housing piece down as far as it would go, then put it all back together.  There is some tension in my steering, but the headset has stayed tight so far.  I did a few curb endos and it is still tight.  Before it would come loose immediately.

Is it possible that the previous owner just didn’t have that black piece right enough???

I don't think the gap is a problem at all.  If you tighten it down without the spring and that's where it goes and spins smoothly, should be good.


Hutch Fun Facts: All main line Hutch's with digits only S/N (no letters or baseballs) on the rear drop outs were US made and had their dropouts stamped between late '83 and '86.  Obviously the frame manufacture would occur slightly after that, even if they just threw 'em in a bucket randomly.  There is an obvious date code in that the first 2 digits seem to be the month and the 3rd digit is pretty obviously the last digit of the year running from '83-'86.

Ok guys, thanks again.  When I tightened it down as far as it would go, the gap obviously decreased.  That may have been the problem.  Previous owner may not have tightened enough.  I just got done riding it again and I beat in it pretty good.  So far, so good.  You can bet your last dollar that I’ll be back in this thread whining about it though, if it comes loose again.

The inside of the spring is supposed to grab the fork, inside the washer groove?  i'll ride more tomorrow and if it comes loose i'll tear it all apart and start from scratch.  Again, I appreciate it.
