How-To Tip, Simple Plastic Brake Washers
Sure you can get these at almost any hardware store, but then they are just a part. These are made with some OS bike pride. 
I cut a plastic soda bottle, and laid the piece under a book to flatten (not really necessary). Circled a dime with a Sharpie, cut with scissors, and used a hole punch for the middle. Simple, and quick if you just need something to work. A little grease helps of course. They are a perfect fit under all the other hardware. You can even get creative, and use colored bottles too.
